Required Documents

For a permanent residence card based on Polish origin, the key importance lies in documents confirming Polish roots, the family lineage, and the applicant’s connection with Poland. After the transition to MOS 2.0, part of the documentation is prepared electronically, while paper originals are, if necessary, certified electronically in advance.

Warsaw

Permanent Residence Card Based on Polish Origin

A permanent residence card based on Polish origin is a permanent residence permit in Poland for foreigners who can document their Polish roots and their connection with Polish culture.

In such cases, not only documents of ancestors are important, but also the entire family lineage: birth certificates, marriage certificates, surname changes, and archival records, as well as sworn translations. If there are differences in spelling of surnames, dates, or places of birth, these must be properly explained and documented in advance.

After the transition to MOS 2.0, the document package must be prepared with the electronic system in mind: scans, signatures, electronic confirmation of paper originals, and communication with the immigration office must all be properly prepared from the very beginning.

SKILLDOCS helps verify eligibility, collect evidence of Polish origin, prepare the application, and support the entire process until the decision is issued.

What a Polish Origin Residence Card Provides

  • Permanent right of residence in Poland;
  • No need to regularly renew a temporary residence permit;
  • Access to the labour market without a separate work permit;
  • Possibility to freely leave and return to Poland while the card is valid;
  • More stable legal status for life, work, family, and future planning;
  • Possibility to apply for Polish citizenship in the future if requirements are met.

  • Power of attorney (pełnomocnictwo) and proof of 17 PLN fee if represented by an agent
  • Additional documents requested by the immigration office in case of discrepancies in names, dates, places of birth, or other inconsistencies

After the introduction of MOS 2.0, paper supplements are no longer accepted at the service counter. If the immigration office requests additional documents, they must be submitted electronically.
