Usually required:

  1. Electronic application via MOS 2.0
  2. Scan or photo of all pages of a valid international passport
  3. Digital photo for MOS 2.0 submission
  4. Proof of payment of the administrative fee (opłata skarbowa 440 zł)
  5. Employment contract for at least 1 year
  6. Employer attachment for the Blue Card application
  7. Employer documents: KRS/CEIDG, NIP, REGON — if required
  8. Proof of qualifications: diploma, diploma supplement, or work experience documents
  9. Certified translations if documents are not in Polish
  10. Proof of health insurance
  11. Proof of residence in Poland (if required)
  12. Documents confirming salary, job position, working hours, and contract duration
  13. Access to Profil Zaufany, qualified electronic signature, or personal signature
  14. Electronically certified copies of paper originals if required

After the launch of MOS 2.0, paper submissions are no longer accepted at the office. If additional documents are requested, they must be submitted electronically through the official system.
